Based on individual’s strength, capability care and support needs we are able to signpost and place individuals in appropriate service through our flexibile step up and step down programme .
We utilise personalised care/support plans to promote independence as well as manage risks.
Our multi-disciplinary team work within the community and pride themselves on respecting the needs, rights, choices of all individuals and encouraging independence. Kem Kare Supported Living starts from the principle that people with learning disabilities have the same fundamental rights as anyone else in society, which includes the right to develop and make informed choices about their lives.
